By checking this box, I represent that I am over 18 years of age, and for good and valuable consideration, the sufficiency of which I hereby acknowledge, grant to Futures Without Violence (FUTURES) and its agents and licensees, the unrestricted and irrevocable right and license to use my name and biographical material, and the right to use any audio or visual depiction of me, and to publish and use my name, likeness, image, voice, and any other materials provided by me to FUTURES (the “Materials”), for my participation in and inclusion in FUTURES’s campaigns or programs, and any advertising or publicity related thereto, throughout the world, in perpetuity.

I agree that the Materials may be edited and otherwise altered at the sole discretion of FUTURES and used in whole or in part in any of FUTURES’s campaigns or programs, in any manner or media now or after known, in perpetuity, throughout the world. I hereby waive any right to inspect or approve the Materials or the use to which the Materials may be applied. I understand that I have no rights to the Materials, or to any benefits derived from them. I understand and agree that the Materials may be made available to publicly accessible internet blogs and websites, including those maintained by FUTURES, as well as social media networks like Facebook and YouTube, and shared across a variety of internet platforms.   

I represent that I have the right, capacity, and authority to enter into this Agreement and that the rights I have granted herein and any Materials supplied by me will not conflict with or violate any commitment or understanding I have with any other person or entity, or violate the rights of any third party.

I hereby release and agree to indemnify and hold harmless FUTURES from and against any and all claims, actions, losses, expenses and liabilities of every kind, including reasonable attorney’s fees, arising out of the inaccuracy or breach of any provision of this Agreement, or otherwise arising out of FUTURES’s exercise of rights granted herein.  I expressly release and agree to indemnify and hold harmless FUTURES from any and all claims arising out of the use of the Materials, including but not limited to liability for libel or slander, or any invasion of any personal property or privacy right.
